Registering with the surgery

You will need to complete a practice new patient questionnaire, as well as our registration forms. These are available in any of our branches. If you have a preferred branch that you would like to attend your appointments at, please indicate on your sign up forms.

Getting a fit (sick) note

If you already have a sick/fit note and wish to extend the period of absence from work, then you can do this by completing an online form. If you are using this method for the first time and want to understand how it works then please see the eConsultations section.

Getting test results

If you have had a blood test, x-ray or scan it is important you find out the result. Whilst we try to contact everyone with abnormal results, you should not assume that no news is good news. Please ensure we have your up to date contact details, especially your mobile phone number and email.

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

Self-referral

Most referrals to other specialists will require you seeing or speaking with a GP, however there are a number of services locally, listed below that you are able to make self-referrals.

Vaccinations

We provide comprehensive vaccination travel health advice and arrange for appropriate immunisations.

Accessing your health record

Patient records are extremely confidential and notes are only shared externally with those involved with the patients care e.g. hospital consultants. Patients must give their consent if information is requested from other organisations e.g. insurance companies.
